We appreciate your physician referring you to Alabama Anesthesiology and Pain Consultants for your pain care. In order to begin your treatment you will need the following:
1. A written referral from your Primary Care Physician to Alabama Anesthesiology and Pain Consultants for the purpose of Chronic Pain Management.
2. The names, addresses, and telephone numbers of your Primary Care Physician and any pain-related specialists that have treated you in the past as well as your last office notes. We will also need the location of any previous diagnostic studies, such as MRI, CT, or X-ray reports.
3. A valid driver’s license.
4. Insurance card
5. Co-pay or Private pay amount.

You may also click on the link for the patient portal to fill out our Patient Registration Form or arrive 15-20 minutes prior to your appointment to complete your new patient paperwork. Also, please be aware there is a $50 fee for no shows.
